Treasurer. 2.1 Prepare and issue a Certification of Registration and Certificate of Authenticity. 2.1 Record/ register the legal instrument duly subscribed by the Municipal Civil Registrar in the Register of Legal Instruments. Assign a registry number. 2.2 Prepare and issue a Certification that the legal instrument has been recorded and registered. 2.3 Pull out the affected civil registry document from file; make the proper annotation in the document and in the civil registry book. For cases of legitimation by subsequent marriage of parents, prepare and issue Amended Certificate of Live Birth of the legitimated child. Otherwise, forward copies of legal instrument to the office of the civil registrar where the affected document was originally registered. 2.4 Issue original/ certified true copies of the duly registered legal instrument, certification of registration, Annotated and Unannotated copy of the subject document to the client.
