2. Payment of corresponding fees at the Office of the Municipal Treasurer. 2. Receive the petition upon finding that the petition and the supporting documents are sufficient and form and in substance. Instruct petitioner for the payment of the prescribed fees by the petitioner.

2.1 Post the petition in the conspicuous place provided for 10 consecutive days. Ensure that publication requirement (for change of First Name, Gender and Date of Birth) is complied. Investigate and consider any third party intervention to the petition. Oaths.

3. Petitioner signs the petition and the verification portion. 3. Act on the petition within five (5) working days after the completion of the posting and/ or publication requirement.

3.1 Endorse the petition and its supporting documents to the OCRG.

3.2 Prepare Certificate of Finality upon receipt of the CRG’s affirmation of the decision. Otherwise, petitioner may file Motion for Reconsideration in case the CRG impugned the decision.

3.3 Make the proper annotation on the subject document and in the civil registry book.

3.4 Issue Certificate of Finality, certified true copies of the Petition and Verification Portion, CRG’s decision, annotated and unannotated copy of the subject document to the successful petitioner.
