CHECKLIST OF REQUIREMENTS WHERE TO SECURE
1. Affidavit of delayed registration starting the name of deceased, facts and circumstances surrounding the death and the reason/s for the delay of registration of the Certificate of Death to be executed by any of the following: the clinic/ hospital administrator where the death occurred; by the attendant at death if death occurred other than the hospital. 2. At least 2 documentary evidences showing facts of death (ex. Picture of tomb, certified true copy of the certificate of burial, etc.) From registering client

1. Submit the duly accomplished Certificate of Death if the deceased died in hospital/ clinic. If the deceased died without medical attendance, report facts and circumstances of death to the EIC and other needed information for the registration. Present documentary evidences supporting the delayed registration. 1. Evaluate the supporting documents; verify records if death was previously registered to avoid double registration.

1.1 If the deceased died with medical attendance: Check four copies of Certificate of Death duly signed by the physician who last attended the deceased.

1.2 If the deceased died without medical attendance: Prepare the Certificate of Death in four copies and ascertain facts regarding the deceased by interviewing the informant/ client. Accomplish Affidavit for Delayed Registration of Death at the back of Cert. of Death.

2. Client proceeds to the Municipal Health Office 2. Instruct client to proceed to the Municipal Health Office. The Municipal Health Officer shall have an approval for registration in the space provided in the Certificate of Death and shall certify as to the cause of death by the client. 3. Payment of the corresponding fees at the Municipal Treasurer’s Office. 3. Ensure the corresponding fees are paid by the client.

3.1. Instruct the client to return after the 10-day posting requirement.

3.2. Comply with the posting requirement. 3.3. Register the Certificate of Death; Assign a register number.
