1. Submit the duly accomplished Certificate of Death if the deceased died in hospital/ clinic. If the deceased died without medical attendance, report facts and circumstances of death to the EIC and other needed information for the registration. 2. Client proceeds to the Municipal Health Office | 2. Instruct client to proceed to the Municipal Health Office.
