      Who may avail: An authorized person is entitled for the available copy issuance of one’s death record once requested.
      CHECKLIST OF REQUIREMENTS WHERE TO SECURE
      1. Name, date and place of death of deceased From an authorized requesting client

      1. Submit name of the deceased and place of death to the EIC.



      Client photocopies the COM to be authenticated.       		1. For Form 2A/2B/2C: EIC verifies in the computer and Processes the Death Certificate (Form 2A) if death record is available. If not available, EIC verifies the Register Book of Death. If still not available, issue Form 2B (Death not Available) and further verification at the PSA should be advised to the client. In case of destroyed/mutilat ed death records, issue Form 2C (Death Destroyed).

      1.1. For Certified True Copy of Death: EIC verifies in the computer for the register number of the COD to be authenticated. Pull out the original copy from file and instruct the client for photocopy of said document.       		None 10 Minutes Emilie P.Mingming RO1

      3. Present copy of the Official Receipt of Payment. 3. For Form 2A/2B/2C: EIC signs the processed Form 2A/2B/2C, indicate OR number.

      3.1. For Certified True Copy of Death: EIC Authenticates photocopies of COD; return original copy to file.

      3.2. Furnish copy of Form 2A/2B/2C/ Certified True Copy of Death duly signed by the Municipal Civil Registrar to the Client.       		None 2 Minutes
