1. Submit original or certified true copy of court decision/ decree and certificate of finality. 1. Check and evaluate court decision/ decree if promulgated by court of jurisdiction within your municipality. 1.1 For court decision/ decree promulgated by court outside the municipality’s jurisdiction, require submission of duly registered copy of the decision, Certificate of Finality and Certification of Registration from the civil registry office where the event occurred.

1.2 Record/ register the decision/ decree in the Register of Court Decisions and certify all pages of the docs. Depending on the bulk of the said docs.

2. Payment of corresponding fees at the Office of the Municipal Treasurer. 2. Instruct applicants for the payment of corresponding fees at the Office of the Municipal Treasurer.

2.1 Prepare and issue a Certification of Registration and Certificate of Authenticity.

2.2 Make the proper annotation in the document and in the civil registry book if the subject document is a part of the civil register. For decree of Adoption, prepare and issue Amended Certificate of Live Birth of the adopted child. Otherwise, forward copies of duly registered court decision/ decree, Certificate of Finality, Certification of Registration, and Certificate of Authenticity to the office civil registrar where the affected document was originally registered.

2.3 Issue certified true copies of the duly registered court decision/ decree, Certificate of Finality, Certification of Registration, Certificate of Authenticity, Annotated and Unannotated copy of the subject document to the client. Endorse to OCRG.
